2011 Grant Evaluations
The Foundation requires an evaluation report to be completed after a grant has been awarded. All 2011 Helping Today grants use the same evaluation form.
It is generally expected that projects be completed within the timelines described below. You may complete and submit the form by its deadline or sooner if the project is completed earlier.
